Understanding TMD (Jaw Joint Disorder)

Temporomandibular disorder (TMD) affects the jaw joint and nearby muscles, often leading to pain, stiffness, clicking, or difficulty moving the jaw. Research suggests that about half of adults will experience at least one short episode of TMD in their lifetime that usually resolves on its own. However, 5–12% of people develop ongoing or severe symptoms that may need professional care.

While dentists and medical doctors are often the first to address jaw problems, more research is highlighting the role of chiropractic care in managing TMD.


How the Jaw Joint Works

The temporomandibular joint (TMJ) connects the jawbone (mandible) to the skull (temporal bone). It’s a highly mobile joint that lets the jaw move up and down, side to side, and forward and backward.

Several muscles control these movements:

  • Masseter & Temporalis – help close the jaw.

  • Medial Pterygoid – assists with closing.

  • Lateral Pterygoid – helps move the jaw forward.

When any of these muscles or tissues don’t function properly, the joint can become painful and restricted.


What Causes TMD?

TMD can appear suddenly after trauma (like whiplash or a direct blow), but more often it develops gradually due to:

  • Teeth grinding or clenching (bruxism)

  • Muscle tension and stress

  • Repetitive microtrauma

  • Joint wear and tear over time

In these cases, the tissues around the joint may send ongoing pain signals to the nervous system. This can make the nervous system more sensitive, causing pain and stiffness even after the tissues have started to heal.


Chiropractic Care and Conservative Treatment

A December 2023 review of treatment guidelines highlighted the value of conservative, non-invasive care for chronic TMD—much of which chiropractors provide. These approaches include:

  • Gentle joint manipulation and mobilization

  • Trigger point therapy

  • Supervised exercises and stretches

These hands-on treatments can improve jaw mobility, reduce pain, and even address the fear of movement (kinesiophobia) or exaggerated pain responses that keep symptoms going.


The Jaw–Neck Connection

Research also shows a strong link between TMD and neck problems. Many people with TMD also experience neck pain or restricted neck movement. Chiropractors are well known for treating these issues.

A May 2023 systematic review found that improving cervical spine mobility can reduce jaw pain, sensitivity, and functional problems. This reinforces the idea that treating the neck alongside the jaw often leads to better and longer-lasting results.
